Not all methods of contact may be available during an emergency/disaster — please choose the one available for existing conditions. Communications sent via contact methods on this page will be directed to emergency operations center staff members. They will do everything possible to get your message to the SLC employee on duty.

- Your name and relationship

- How you can be reached at that moment (email, text message, cellular, landline, etc.)
